Occlusion of peripheral pulmonary vascular bed in a baby with idiopathic persistent fetal circulation.

چکیده
Babies with a normal heart and persistence of the fetal circulation are frequently referred to the cardiologist because they present with severe cyanosis in the absence of parenchymal lung disease. Persistence of the fetal circulation may be associated with birth asphyxia, polycythaemia, hypoglycaemia,l and premature closure of the ductus arteriosus.2 Occasionally, however, no "trigger" factor can be identified. The present report concerns such an infant who died at 8 days, despite optimal medical management, because the peripheral pulmonary arterial bed was occluded by smooth muscle and swollen endothelial cells. The development of the pulmonary circulation was assessed by applying quantitative morphometric techniques to the injected and inflated lung. The structural changes found were more severe and extensive than those previously reported.3 4

Morphologic development of the pulmonary vascular bed in experimental coarctation of the aorta.
Although electrocardiographic evidence of right ventricular hypertrophy is considered common in newborn infants with coarctation of the aorta, the reason for this finding is not well established. Investigations of the pulmonary vascular bed of these infants have resulted in variable findings, probably due to the differences in morphometric techniques, coexisting cardiac defects, and variable po...
متن کاملPreoperative partitioning of pulmonary vascular resistance correlates with early outcome after thromboendarterectomy for chronic thromboembolic pulmonary hypertension.
BACKGROUND Pulmonary thromboendarterectomy (PTE) is the preferred treatment for chronic thromboembolic pulmonary hypertension (CTEPH), but persistent pulmonary hypertension after PTE, as a result of either inaccessible distal thrombotic material or coexistent intrinsic small-vessel disease, remains a major determinant of poor outcome. Conventional preoperative evaluation is unreliable in identi...
متن کاملInactivation of bradykinin in the pulmonary vascular bed of newborn and fetal lambs.
Bradykinin is largely inactivated in one passage through the pulmonary vascular bed of adult animals. We studied pulmonary inactivation of bradykinin in newborn lambs, fetal lambs at term, fetal lambs preterm, and adult ewes by comparing the systemic pressure response to infusions proximal to the pulmonary vascular bed with the response to infusions distal to the pulmonary vascular bed. A high ...
متن کاملEffects of Eicosapentaenoic Acid (EPA) and Docosahexaenoic Acid (DHA) on Fetal Pulmonary Circulation: An Experimental Study in Fetal Lambs
Background: Persistent pulmonary hypertension of the newborn (PPHN) causes significant morbidity and mortality in neonates. n-3 Poly-unsaturated fatty acids have vasodilatory properties in the perinatal lung. We studied the circulatory effects of eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) in fetal sheep and in fetal pulmonary arterial rings. Methods: At 128 days of gestation, ca...

bed in which we deduce that hydrostatic pressure is elevated. In the newborn lamb, the results suggest a lower capillary pressure and larger capillary bed than in the fetus. The pulmonary vascular bed of the fetus, we conclude, may contain large unperfused areas; and vasodilation at birth may take place mainly by recruitment of additional areas. There is support for these ideas in the very nonl...
